Drug/Alcohol Questionnaire

Are you concerned about your son or daughter’s behaviour recently? Below are ten questions that parents often ask if they are concerned about their son or daughter using drugs/alcohol. If you notice a number of similarities between these questions and recent behaviour changes in your child, you might want to ask them how things are going.

Most young people who begin using alcohol /drugs display a number of these behaviours. If you are concerned, take the quiz and compare your perception of your child’s behaviour to those listed below. If you get a lot of “yes” answers, it may not mean your child is using. But if you are concerned and get a lot of “yes” answers, you can call us for more information.






1. Are curfews suddenly a problem for this child?

2. Have you detected a "who cares attitude" in a child who was once involved in different teen activities?

3. Is there a secretiveness about this child’s life that is beyond what you think is normal for an adolescent?

4. Are there unresolved questions about money or unusual requests for money, or its unexplained loss?

5. Are there phone calls for this child where the caller hangs up when you ask who it is?

6. Has this child’s sleeping and eating pattern changed beyond what you what you think is normal for an adolescent?

7. Has interest declined in school or extracurricular activities, or sports been dropped?

8. Does your liquor supply seem to go down too quickly?

9. Has your child’s personality changed in extraordinary ways ie. outbursts, hostility, or extreme calm?

10. Has your child changed from friends you knew well to more secret relationships?
