To access our VIRTUAL Walk-in clinic, please complete the sign-up form below and a counsellor will call you at the next available appointment slot.

Please note that our caller ID comes up Private Number, NO CALLER ID or 1-888 depending on your carrier.

If you are unable to attend, or you miss the call, you will need to reapply.
